Badger Teemo Auto-Attack Visual Bug and Frost Queen active stuck in place

In a game I played yesterday I noticed a badger teemo in the mid lane auto attacking the champion and minions, then out of the fog of ward behind him another instance of teemo's brown auto attack flew straight down the lane and vanished.

The Frost Queen bug that I am reporting has occured on several occasions when I play support. I could be standing in the middle of the lane, river, or have my champ model next to a wall with branching paths and decide to use the frostqueen active and watch the ghost use it's moving animation yet the ghost actual does not move at all.
