Sion Glitch?

Was doing an ARAM with Sion in a league game. Everything was fine and dandy tile suddenly my Q and my R wouldn't activate.

It happened in a point to where i was activating my Q while i got killed, Don't really know what happened next cause it was all a big pile up after that. Was lucky The team won but during that entire game neither of my skills didn't activate. I'm mainly thinking it's a bug of some kind and might need to be looked into. I'd report it on PVP.net like what the help button said on league but I have NO idea where it's at :Y
