I keep pinging to back or be careful and it is producing a general anonymous ping or - worse - an attack ping, causing lots of confusion and lost games.

Agreed, v+click is now blue ping like g+click and there's no convenient button to open yellow ping target. It messes with the flow of players gameplay and how they've played the game for years.
yup, the retreat ping is not working. Its all default to pressing only v, but it only working when I press ctrl+v and it doesn't work all the time.

Yeah same here, pls fix it!
ctrl+click is working for me. they probably accidentally changed the default ping settings, which means it could be fixed by going into hotkeys.
