Q W E R Randomly not working

So Ive noticed an annoying bug in Player vs AI on Summoners Rift (don't know if its spread from there) in which some combination of Q, W, E, or R randomly stops working during the game. Both for myself and for other players. Ive tried assigning the button to other hot keys and pressing the ability directly but it just wont activate. This happened to me as VI where i couldn't use my Q (This occurred for me at 16:00 minutes into the game )and to a Sion team mate the next game to his Q and R (or W and R). My Q cleared up the next game and has since not caused me further issue but my team mate Sions did.

Note that it still makes a sound when you press your button (I press Q and receive a 'click' confirmation in game for pressing it) but it does nothing otherwise. The same is true for clicking it in game with the mouse.

Anyone else having this issue? if so was it only in Player vs AI?
