Getting killed as Fiora turns off summoner spells.

I was just playing a game vs a Darius. Darius was the enemy top laner. A few of the times when Darius killed me it turned off my teleport and flash. The bug seems fairly similar to prerework Fiora when you got killed in ult and it did the same thing. I'm not sure if it would be related in anyway. Sometimes he killed me and it turned them off sometimes not. It happened maybe 3-4 times during the match. The summoner spells enabled once I auto attacked minions every time.

This is happening on patch 6.9
