Ranked Que Glitch

I queued up for a ranked game, we get to picking champs and it comes to our team's second turn to pick, one guy didn't pick a champion and the timer hit 0, but the queue didn't disband, he was still talking with the team and it sounded like he picked someone. After a while I got an error message and I got sent back to the main menu, I got put into a delayed queue and lost LP for "dodging". Is this something that is common? Because I don't want to lose LP because of a glitch I can't control.
