Game crashed, kicked from match?

I was playing a match as Aatrox, winning lane, pushed to inner turret. Just before I killed the inner turret, my game froze, and crashed. BugSplat bug report froze and crashed, and then made me send in two of the same bug reports. League stayed open, but nothing happened when I tried to maximize it from the task bar. Close it, and then reopened, and relogged. Instead of making me re-join the game, the game appears to have ended instead, and I got a teamwork honor. Wasn't at 20 minutes yet, around 16, so they didn't FF during the 1 or 2 minutes I was out. Got a warning to "Not leave anymore games" and was made to sign a contract saying I wouldn't leave anymore. This is the second time I've had to sign it. First I had to because I started a game at around 12 PM, and was told that I would have to go around 3 or 4, so I just was going to play a game, but ended having to go at around 12:20 instead, and so I had to leave the game. I think I may have broken the game.
