So. This was a thing for awhile. I don't know why but I have two originals of Vel'koz.
It's not much of a problem but to me I find this hilarious. Question is. Which vel koz is better? The one on the left or the one on the right?
Nami had the same bug for a long time too, but I think it's gone now that Urf the Nami-Tee came out.

For me at least, its been a thing since he came out.
I had this glitch a little before vel'koz new skin came out. So I doubt it's BECAUSE of a new skin.
I'd imagine it probably means that a new skin is coming soon for that champion, and a placeholder image was wrongly packaged with the game files. Just a thought.
Bumping this in hopes that a Staff would see this glitch.
