This happened twice to me that game. My mouse was hovered over Ezreal both times I pressed W and R, but my ult shot almost backwards.

Krigjer1 votesI got this on Nami. Threw a bubble behind me, went right in front of me, despite mouse position being where I aimed it, not where it landed.
You can say 'lol aim better scrub' but I know what I saw.
