Skills randomly going on CD

I've been playing league since season 1 over the course of time I've played league i have noticed this bug probably 40 or 50 times. its probably happened more but I usually chalk it up to human error or something else. I once asked a rioter if i should report it and he told me that it probably wouldn't matter if i didn't have proof or if I was unable to remake the bug. Well I don't know why it happens or how. I don't even know if there are ever any similarities between the times its happened. It wasn't until my last game, with ahri, that I knew for sure it wasn't just me being absent minded.

Basically the bug is this. I will generally be fighting or about to fight someone in the jungle or just before a team fight. I will check my ult or flash cool downs to see if they are down. After making sure they are up i will enter a team fight, and attempt to use that skill only for it to just not activate. Upon looking again, I will notice it is on cool down. I'm sure this happens to other people, and that's why i'm posting it on the boards.

Todays Ahri example; My team mate was fighting a Morgana in mid lane. I checked to see if my ult was on cool down with the intent of using the first charge to kill Morgana, and the next 2 to possibly escape anyone who may be close enough to help her. (as we had spotted Rengar not long before) When i came within range I pressed R and nothing happened. I tried a couple more times thinking maybe i didn't press it, before realizing it was in fact on a 110 second cool down. I ended up just using q to kill her instead. At first, I was thinking about when i could have possibly used it and couldn't remember doing so. It was also rather confusing because even if i had accidentally used it coming down lane or something I would still have 2 charges and the cdr would be for charges not the skill right? That's when i realized that the actual cool down on my ultimate was only 95 seconds with 10% CDR not 110. In the past I have noticed that sometimes it will be a seemingly shorter or longer cool down. An example being, flash will just randomly go on a 6 second cool down instead of full or something along those lines. Again i usually just chalk it up to my error, but I'm pretty sure that's not always the case.

In the past, upon checking replays, during the times in which I've experienced this, the replays show the skills as not being on cool down at all during the entirety of said fight or attempt to use the skill. In the end, the only real way i would be able to provide proof would be to stream and record said game at the same time and put them next to each other to show that they are different. Thus far I have failed to capture it in both at the same time as it is seemingly random.

I tried to google this to see if anyone else had posted about it in the past on YouTube or the boards or anything and couldn't find it so I assumed this would be the best place to see if I'm not the only one.

If you happen to have an Nvidia GTX 650 or higher video card, I suggest enabling Shadowplay, which will allow you to record up to the last 20 minutes upon request (at virtually no performance hit). I don't know if AMD has an equivalent, but third party applications probably do not do as well as Shadowplay because the GPU handles all the work directly. This is great for recording things on a whim without recording everything and filling up your hard drive. (it just constantly records but overwrites itself)
