Guardian Angel not working.
Unsure if it was Jhin 4th shot, the flash, the combination or runes.
I was pretty sure it was up during the game so I checked the replay to make sure. Below is a video of it, I can upload the entire replay file if needed.
We lost either way and even had I revived we wouldn't have won. Regardless this is a bug.
