how/why do these inconsistencies get through?

this isnt really meant as a flame post but more of a general question. since i am a freshman in collage who is majoring in CS with a concentration in game design im just wondering how the artistic process of champions work out. Since the first thing you see about a champion or a new skin is the splash art, i feel like that is what should designate the design of the 3d model and the animations to go with it. Even of these inconsistencies arent really that important in terms of gameplay, they are still important in conveying the character and how they move and whatnot. And with the Kindred release and Wolfs mask missing the ears, or Lamb having 5 fingers in the splash but 3 in game and all. and the masked shaco thing.

Maybe the ears muddled the model and thus were not included.

Maybe the ears were added later on on the art, and the effort required to change them in the model was deemed too high.

On the hands, it might be that they were running out of time and decided that three fingers were enough (all animations in LoL are made by hand and are very time-consuming).
