Creepiest would probably be Orianna 
.
She's a living doll made in the visage of a man's dead daughter, nothing more than a simulacrum. Yet her attempts at being alive are just imitation, some thing trying to wear a mask you and I are born with. This is distinct from the primal terrors like Nocturne or Shaco, who are malevolent spirits/creatures on their own ... not broken replicas.
In the same vein, she's also incredibly tragic, because she'll never be fully complete, either.
is in my opinion the most horrifying,since he is a sadistic psychopath,who will torture you physically and mentally before he kills you and once you are dead he'll torture your soul just for fun.
But I dunno if creepy is the right word for that.
