Now correct me if I'm wrong, but as far as I know the Omega Squad is like Bandle City's elite special forces. Since it's Bandle City which is supposedly only accessible by Yordles, it doesn't make much sense to include Fizz and Twitch in them, since they aren't Yordles. Also, Veigar isn't exactly fighting on anyone's side, since he's The Tiny Master of Evil. Furthermore, in Veigar's bio it states that he frequently went on missions with a business that frequently traded with the other city-states in Valoran, so he wouldn't have ever had time to be in the Omega Squad.

I've also heard that the rest of the Omega Squad is supposed to be dead, leaving Teemo to be the last member.

the Omega Squad skins are set in a parallel universe, so not cannon

Almost every skin is set in an alternate reality or at least a different time.

I mean Galio is a demacian statue and not Hells Gatekeeper during his spare time.

Considering that the Omega Squad artwork looks somewhat dated resembling older camera quality and Teemo is in a lot of those splashes, but without his Omega Squad appearence, there is the theory that this skinline shows the Omega Squad before Teemo became the sole survivor.

Omega squad is in a alternate universe. My personal theories involve the Omega Squad forming after the rise of the Battlecast, Project Initiative, and Mecha skin line. This could either mean veigar technically isn't veigar, and twitch technically isn't twitch just for the sake of this fictional war-film-esque universe. Or it could be in that alternate universe I like to think is real, and veigar casts aside his lil' evil tendencies against a greater foe.
