so as far as i know, they biggest lands are demacia and noxus, but there is also piltover, frejlord, the void and many more. But how do they connect, do they have a deeper meaning? wich are fighting who?, can someone give me an answer???+

Who knows anymore? Nothing in the past is really canon anymore so for all we know Demacia is now Noxus-Incognito and Noxus is now Ionia's play-pen and Ionia is now to Demacia as Belgium was to Germany.
Seriously, make up whatever you want. At the rate Riot is pushing out lore you won't be wrong for at LEAST another 3 years.

Technically, the Void is not a part of Runeterra, or even a "land". Its a space between dimensions.
There are 4 large nations that govern most of the territory of Valoran: Freljord (North), Shurima (South), Demacia (West), Noxus (East). There are 3 Island Nations: Bilgewater, Ionia, Shadow Isles. And there are dozens of nations or city-states that govern a very small portion of Valoran, they are ether a nation within a nation, or coastal nations: Bandle City, Piltover, Zaun, Mount Targon and its tribes...
There are several realms that can be accessed through certain portals that are located within Runeterra: Fae, Void, Celestial grove. These realms do not function in the same way as Runeterra does, so time and space is distorted there.
And there are also the interstellar worlds, but we have no information about those only the hunch that there are other life sustaining planets out there (Celestial Beings needed to come from somewhere).
