Are all the Ascendants supposed to be animals?
Renekton, Azir and Nasus are all animals, so why isn't Xerath? Did he goof on the ritual or something?

Yeah,Xerath used his knowledge about Magic to manipulate the ascension ritual,so that he can take it to much greater lenghts than the other ones.
This caused him to become a being of pure power(but he got trapped in the sarcophagus by Renekton and Nasus,before he could reach his full potential) and that is why the ascension ritual caused such a large amount of destruction in Shurima.

During the ritual, the person to go through with it wears animal theme garb.
If you read "Unbound" which is Xerath's lore piece, it mentioned that Azir had on a falcon helmet, so Azir's ascended form was pre-selected and not chosen at random.
Same could go for the Ascended Brothers. Nasus chose the appearance of a dog and Renekton the crocodile
Xerath doesn't have a form because he didn't participate in that one part of the ritual.
Because it was a role he was never meant to fit into. He was shoehorned into it with the retcon.
It really is that simple.
Xerath interfered with the ascension ritual, goofing up mystical mumbo jumbo in the process. Somehow this translates into him trying to super ascend into a being of pure energy, but got thrown into a coffin instead with an alligator for a room mate.
In general it seems the typical shuriman ascension instills animal traits/form, though why isn't clear.
Xerath was a mortal that obtained power and lost physical form correct? Don't remember but that would make sense.
On a side note, is 
an Ascendant?
All others earned their ascendance. If i am correct, xerath somehow artificially created his ascendancy, he did not earn it like the others.
He did sabotage it before hand. Who knows what he did to it.
Xerath changed it to gain more power.
