Hello, I just reported a player for threating to gut me, or kill a family member. And I would like to keep track or know for sure that this player would get banned hopefully riot would ban his IP address.
About everyone saying you'll get a notification, you only get this if your report was the last straw which resulted in the punishment. If he's punished the very next game, you won't get it.
Riot won't otherwise inform you about punishments to other players, so just put this behind you; this kind of behavior is definitely a permaban offense so just leave it to Riot and don't dwell on it.
You get a notification in your client when punishment has been given to a report. It doesn't tell you which one, just that it has been dealt with.
Riot doesn't give a play-by-play on any reports, in-game or on their web support. Actually they say:
For the sake of privacy, we cannot disclose any disciplinary action that may be taken, but we will investigate.

Riot doesn't do IP bans at all that I'm aware of. That being said they do take threats very seriously, you'll get notified if he gets punished through a pop up in your client.
It's the internet. Stuff like this sucks, but it happens. And they won't give you information on who is banned, the most that will happen is you'll receive a notification that a player you recently reported has had actions taken against them.
Not sure what banning an IP address would do.
I would report this to your local police
