So let me get this straight...

So in a game I have a Khazix. Right off the bat he sees them pick Ivern and goes "That's GG guys, please dodge or remake.' We tell him it will be all right. He continues moaning about it into minions spawning. He tries a bad gank mid, and dies says "That's why I wanted a remake." Does another bad gank dies again and proceeds to go off on us being noobs and trolls. Declares he's going to go AFK. Doesn't, but then gives up 3 more kills, continuing to call us noobs and trolls. A couple of us get annoyed and call him on it, stating that he is making bad decisions. He then proceeds to intentionally feed. As in walking down lane into the enemy, not attacking or anything, just walking past as they attack. He does this repeatedly. We all agree he is intentionally feeding. The enemy team agrees he is intentionally feeding. At the end of game he says he is reporting EVERY PLAYER IN THE GAME and e-mailing cropped footage to prove his point. We ALL report him for intentional feeding and unsportsman as he continues to berate us for being trolls. I take a break, come back 2 hours later and I've been punished, but received no confirmation that he has been?

Is this for real? The cry baby trolls who intentionally feed get their way? I'm honestly blown away at this. I guess instead of taking the high road in games when people are toxic and such like Riot even suggests, I should flame everyone, then crop footage and make erroneous reports to Riot because THAT gets far better results.

You don't get punished after being reported in one game. Punishment is automatic now, and happens when you get a high volume of reports over a certain number of games.

Some of these guys get it in their head that "Champ X is OP and you can't beat them", and their solution is "If we can get fed, we might win" so they take these massive gambits, obviously eat it, and then go screaming bloody murder - reinforcing their own philosophy.

There are some champs I do think "ah crap" but honestly, punish other lanes, scoop up towers, and let the super minions carry you to victory. It's a childrens game. Sub-18 year olds play and do well at this. It's not Brain Surgery. Some of these guys are so KDA focused they can't see the nexus through the minions.

Depending on how you responded to him trolling, you could easily get punished as well. A lot of people think "we were responding to him!" makes them immune for their tone or reactions. If it was a chat restriction, you should've received logs you can post for us so we can identify why you got punished. plus INT feeding is harder to catch than people think, if someone does it just one game and all their other games are fine, then the flag goes unnoticed.

Or if they only called you noobs/trolls and such without being explicit or using terms that are looked for in the automated system, then if it was just them doing it one game out of many, then it can easily slip the net also since it takes a few games getting flagged for the system to take action against someone.

{quoted}...but received no confirmation that he has been?

I believe (might be wrong) that you only receive confirmation if you report a player in the specific game that triggers their punishment. It's possible that player isn't super frequent with their toxicity, and thus still has a little wiggle room before the system applies a punishment. Or equally possible that they're reported and punished after their very next game, in which case you still would not get confirmation.

If you called out to report him in the game either in /all chat or in team chat, that is a punishable offense. One report is the same as 9x, they do not stack and they are not 'weighed' more by Riot. One is enough, and you do not need to threaten other players in chat with "Report ____".

If you were verbally abusive in any way back to the player, that is punishable. Two wrongs don't make a right, and you cannot "fight" toxicity with more toxicity, even if you feel justified in doing so.

Here's what you SHOULD have done: say nothing in chat, and just report the player end of game.

So my overall take away from this is that apart from pings I should just no longer communicate with my team, since it is far too easy to construe things as a reportable offense.
