It took me a couple weeks to get my rewards back, but I did. And this was after a 2 week ban, too.
it does, although riot still has not given out information to the public on when you are considered "reformed", but it's usually a week after a 10 game chat restriction
Took me about 70 games to be able to earn rewards. That was after a 2 month break after being banned
