Where do our reports even go? (Riot response please)

I just want to know what happens when we report trolls and others that cuss out our mothers etc etc. Every Rioter says that only one report is required to punish someone but this is obviously not true. In about half my games i get either a troll or someone who just flames everyone. For example last game i was supporting a MF who went underneath the enemy tower at lvl 1 and died. She then proceeded to troll and eventually just went afk in the base. In the that match we had a Yorick top who flamed our Shyvanna calling her mom "garbage" and calling her "garbage's son". I reported both of these people for their offenses, but I know that they will go unpunished. After playing league for years, I've only ever had Riot say that they punished two people that I've reported. If it is true that each report can get someone punished, then why doesn't that happen? Why will these people go unpunished and be allowed to do this in other games. And why am I losing LP when there is a troll on my team. Why won't it be loss prevented?

They are, all the time. People are here on the boards 100 times per day whining about being banned. Perhaps the cases in which you reported people it wasnt severe enough to warrant an instant punishment. I get the report feedback quite often after i report someone who was saying terrible things in game.

Automated system. Pretty much works via flags, and yes one report is enough to make the system look for red flags such as hate speech, spam, abuse, curse words etc.
