Are the punishment and feedback systems broken?

Just had an especially toxic game with a guy who ran the whole gamut of bad behavior. Flaming, AFK, aiding the enemy team, even telling everyone to go kill themselves and arguing with the enemy team. One would thing it's a sure thing for a ban, yet it's been over a half an hour and I still don't have any notifications regarding that. So either he's been banned and the system is busted, or he hasn't been ... and the system is busted.
