I've had connectivity issues lately and this has resulted in me d/cing from a few games in the last week or two. Leaverbuster punished me as a result by putting me into a lower priority queue for 5 games. I have never been disciplined in LoL before - does this mean I won't be eligible for ranked rewards at the end of the season, or would that only happen if my account were suspended?

I'll start off by saying no, you are not going to be ineligible for 2016 season rewards for getting low priority queues. Last season you had to have been account banned within 3 months of the season's end, or chat restricted when rewards were given out to be ineligible. We are a very long ways away from that, and LPQ's from Leaver Buster didn't count anyway.
