Way to report with video?
Hey I had someone that was intentionally losing my last game but it's hard to show just by end game score. I do have a clip of him doing it though. Is there a way to report through video submission

The best thing to do in this case is describe the actions the other player was taking that you feel are reportable in the report screen, with time (approximate) if possible.
If you're still wanting to send more information, you're welcome to submit a support ticket (say: "I'd like to report other abusive behavior"). They won't tell you what (if any) action was taken, but they'll review and take appropriate action on the other player's account.
Yes actually; the best thing I can suggest is going to the match history in the client and download the replay file. You can then send that .rofl file through a support ticket for Riot to view over.
Now here's the kicker though; not only will you not be told the results of their decision, but 90% of the time you can OP.GG the player later on to see them still playing, punishment free. But it's still worth a shot I'd say, maybe Riot will actually for once work at their toxicity problem.

While others have already mentioned methods of reporting with videos, it really isn't necessary.
Riot already has access to all the gameplay videos with more details than you think; they have access to click speeds, click locations, what and when each button is pressed, whether skills were leveled up, ping / Lag etc etc.
These data is what they refer to as "Gameplay DNA" and everyone has their unique DNA. Which is how Riot is able to tell which are alternative accounts of certain players.

I just got banned because i flamed at REGNAR who was just sitting in the bush watching me fight jax, but b4 all that i just saved his life in the jungle so he said he was coming top pinged and evrything but went over to steal the jungle KRUGS. I was mad then agaian he comes top and just watches me and jax fight, if he had jumped in we wouldve had a chance. I'M A MAN DONT PUT ME WITH KIDS.
