If that option isn't available, you should instead make a support ticket: https://support.riotgames.com/hc/en-us/requests/new .

Modi1 votesYou can do what TheEvilQueen135 said. I have seen good response from support regarding both summoner and club names with support. The other option is to report post-game as a "summoner name". I was told by support that they check both the player and club names for those reports, but you can add a comment in the post-game report that says "club name" or similar.
