At which cases does the system skip the 14days ban? And is it possible to play an x amount of games and be banned (permanently) for a game that happened before the 14 days ban? Thanks in advance

In "extreme cases" you can go straight to perma. This is at riots discretion but generally involves racisim, sexism, or telling someone to kill themselves.
For part 2 of your question i would say you likely will not be punished for a game that happened before your current disiplinary action was issued as that case was already taken into consideration for whatever punishment tier you may / may not be on.
