can someone link me to the page so i can report another player to riot support??

i just lost a ranked game single handedly due to an ap graves support who didnt speak all game he just afk farmed botlane and refused to help the team the entire game, never left bot once, when he saw we was still winning and taking bot inhib he then afk farmed in the jungle the rest of the game to not push lanes anymore, we couldn't end because it was a constant 4v5 and the enemies eventually out scaled us and we lost. we even had the enemy nexus open at one point but our hecarim didnt have enough attack speed to take it fast enought, he tried and graves was right there and blatantly refused to hit the nexus and just farm minions instead. We lost a good game due to a single player. this is why no one can carry games anymore.

please i want to make sure this guy can ruin anyone elses games someone please link me to where i can write a report out for him, i cant seem to find it.
