I'm sure they've run into many warnings before they get Permanent Banned. so how do they even still manage to fk up and say stupid shit like bad sportsmanship.

some people just don't get it. They think what they say isn't that bad and they just end up blaming others for their behavior.
I mean I find it's pretty sad. Especially with how much times they are warned about it, just seems weird to not take the hint. But from being on this side of the board for years I can say in most cases people just don't care. They see the warning but they think they've done nothing wrong. I've seen people say basically every single slur in the book and still claim they were innocent. It's pathetic!
I have no idea, I've left a few games (not often but it happens, sometimes due to something I can't control sometimes by something I can), I've raged and flamed over chat (not often but sometimes unless you just mute everyone at the begining of the match it's hard not to be dragged in by a troll) and I've intentionally trolled a team of shitty people before. None of these are habits. I think ive only trolled/left intentionally a hand full of times since the original release of LoL but I've never been banned, not for a day. So it confuses me when folks say they've only ever left once or flamed once and now they are permabanned. In fact I find that incredibly hard to believe.

Some people just don't think what they do is bad. That's how they get permabanned.
