Why don't people who deserve punishment get it?

A lot of people in game really do deserve a punishment, but a lot of the time the don't get any (I don't think, because I don't get a notification, but please correct me if I'm wrong). For example, these two duo's were feeding and trolling at bot lane. First they stole bot lane (yas and sion), and proceeded to feed and was complete butts about everything. I reported them, but to my knowledge they didn't get any punishment. Another time is when toxic people flame you in ranked and become extremely toxic, and yet again they don't get any punishment. I've only ever gotten the notification once, and never again. But I'm not entirely sure if these players will get punished without sending me a notification. Maybe someone else reported the person and the person got punished, but I get kinda pissed thinking that these complete butts are getting off scott - free. I'd really want more notifications on this matter, and just knowing that these people learned their lessons (I hope) would lift a small weight off my chest, as I know that the community of this game is becoming a little less tilting enough to make me quit.

you only get the notification when your report is the final straw that banned them.

You won't get it if it was building towards their case.

Because the system isn't perfect and outlaws find their ways around the system, same as our world. If anything, the system shouldn't even be needed and we rely on faith in humanity's honesty to keep our order here, but unfortunate that isn't happening so the system is brought out.

The fact that you see toxicity in game is not evidence that the toxicity is not punished. You can't punish people BEFORE they do something toxic.
