And he was even punished for the trolling/griefing we also reported him for, which he admitted to in chat.... what is this game?

edit: I also have his account saved on op.gg, to keep tabs on him.... so I know 100% he just went right back to doing his thing.

I'm not sure how/where you know when a player's name is changed. It might show on op.gg, but for sure, their match history changes on the LoL website. Their board profile may not necessarily change.

Forced name changes take a while to propagate even to Riot websites such as the boards. Third party sites such as op.gg are very bad at updating the site for name changes.
