Thats the thing with these people, death threats, racial slurs, doesnt mean shit because they were reported an excessive amount of times. Since the release of dynamic queue bans have sky rocketed, coincidence? i think not. its because premades can report someone to get them to catch a ban. its a flaw in the system that needs to be pointed out and fixed
Well, either he doesn't do it often or people aren't reporting him enough. Players must show a pattern of a negative behavior to get punished.
