I just got kicked from a Lobby for not banning a champion. Then got a 15 minute lock out for "queue dodging".
It was my second game of the day. I played, and won, the first.
So I have to ban champions or I get kicked out?

No, the client treats that as a dodge, just like how you have to lock-in a champion before the timer runs out. There's a "none" option if you don't want to ban anything, but you certainly have to lock-in something, even if it's "none"
You have to lock in something for a ban. There is an option for “no ban” but you have to click it first. It is just a good way to force dodge afks instead of waiting until last pick for the dodge
