Find a tattoo artist who's style you like. Show him a few screens hots and splash arts and ask him to create something.
That's the only way to make it truly unique.

Most tattoo artists want to create their own pieces of art for you. All you need to do is find an artist whose art you like, make sure its from a clean and respectable shop, and provide reference material as well as what size/budget you're looking at.
