Help make Champion Concepts

I was trying to make a champion concept, but I didn't know what all the numbers meant. (12/34/56/12/60) I know most of the AD and AP stuff mean. Also, when posting my idea, do I put the lore in there as well? Or not? If you can help me understand, it would be much appreciated! Thank you!
