Submit a support ticket, you should get an email
There's no eMail probably because there isn't a justifiable reason other then playing the game and just happening to be dying a lot which appears to be intentional feeding.
Do you remember the e-mail for this account? Check spam....If you really want to see the logs Ask @Riot in Player Behavior and see if you get an answer. As I'd like to see the logs also, tag me somehow and I'll try to draw attention to your post...GL
to be honest alot of your games look like intentional feeding.
