Hey, I'd like Riot to take a look at my account and make sure it's not hacked, because I haven't said anything but niceties or nothing at all the last 20+ games, not to mention I haven't even had all-chat on at all.
I'm getting so friggin annoyed that I keep getting these things just because people randomly report me for no good reason and the BOT just goes 1+1+1 = fun.

Something is just not adding up. I would assume like most bans they give you a reform card. If you have such thing in your email by all means put it here so we the community can see. Not saying you are lying, but usually for rank ban I believe it's something like leaving games too often, and chat bans are usually from getting reported because of your language.
I would say to contact support. Usually they can have someone actually look up to see what's up. I mean slip ups can happen and maybe you got a ban when it was for someone else!
