His r has to be one of the most toxic abilities in the game.

Karthus has a lot of problems. He is a very weak laner vs burst champions Zed and Ahri for example. He is also very immobile with only his slow as a way to try and run away. His ultimate is strong along with his passive in the late game, however a number of skills, items, barrier and heal can all be used to soak up or out right stop the damage. The real best use of the ult is that it can clean up anyone running away low on health helping to snowball another lane. Still that lane needs to be winning for this to really get the ball rolling. I find I do not see him very often anymore, as he has fallen out of favor and is no longer in the upper tier of mid laners.
