This has been a problem forever, and it has a couple very simple (if partial) solutions that Riot always resisted implementing:
1) Allow players to queue for the lane they want - This is the ONLY way to solve the problem of players not getting what they want. You can come up with systems to determine who's right and who's wrong, but that game is ruined either way.
2) Allow players to choose a solo-only queue - This helps avoid the duo queue ganging up on someone, the duo queue trying to pick out of order (because they want to lane together), and honestly really helps solo queue players who want to lane bottom.
3) Allow a votekick or reporting in champ select - Players shouldn't have to play a terrible game out and take an MMR hit just to report someone for being a dick. Any system like these would help, but you're still gonna have complications in addressing the problem at this level rather than at the start (point #1)
Ranked Team Builder is coming soon, but of course, this type of behaviour has got to stop.
The Player Behaviour team, and Riot Lyte, seriously needs to take this type of situation seriously. They should not judge a player being bad & evil just by:
- looking at the number of reports onto him.
- punishing the toxic guy who was actually innocent, and leave the toxic guy who was the real criminal that ruined the whole game free.
I really wish the guy who trolled Sky deserves to get demoted 2 divisions down.
Not a big fan of Sky but when he is right, he is right.

Just realized something.
Look at 1:05. THAT was Sky's troller in his queue.
Tried to add him to tell him off how much of a dick he was, and I got this:

In the youtube comments, his username was already leaked to everyone viewing Sky's video. Guess the dick got way too much hate letters filling his box. He deserved it :p
