I haven't really played for 2 years. Is it worth getting back into? if yes, can someone give me a rundown of all that's changed?

I think it's a pretty fun game. I play it all the time when I have nothing better to do.
If you got some free time and possibly (but not necessary) some friends, I don't see why not try it.
Although I started a little over a year ago. A big change they made is that the masteries at the furthest portion, is usually something stronger that you can see in your games.

If you can enjoy playing a competitive game, for whatever reason, then this is still worth playing. If you play and get upset because bad things happen, then it's probably best to play something else for your enjoyment.
On changes - new champs have been released (about 5-6 per year) that are pretty cool. Some are better than others in the current meta. There is now an official rotating game mode on the weekends that brings some more fun game modes out from time to time (everything from URF to Hexakill) Several older champions have gotten full reworks, adjusting visuals and abilities to meet current standards. Most recent is Galio, but also Sion, Poppy, Taric, and a few others. General changes to the jungle, masteries, items, and so on as usual. Jungle now has little plants that grow and do different things. They're kinda neat. Dragon has had its buffs changed - there are now 4 different dragons that can spawn, each giving a different buff. Scuttle Crabs patrol the river. Honestly can't remember how long ago they were introduced so you might already know them. Dominion has been retired as a normal game mode and integrated into the Rotating Game Mode queue. Lore has been updated for many champions and regions. Check out the universe link at the top to explore all that. Class updates - larger scale changes aimed at a group of champions (like mages or marksmen) that aim to increase diversity within the group. So far, mages, marksmen and assassins have been updated. New classifications of champions, such as juggernauts or wardens, exist to break down more generic classifications like tanks.
Upcoming changes (that we kinda know about) Urgot and Eve are next in line to get visual-gameplay updates Noxian lore will probably launch when Urgot is released (as they just did Galio's rework and Demacian lore update) Runes are being discussed internally. Many people are unhappy with the current system. Class update for one of the tank classes (Vanguads or Wardens)
