Confused on How to get the Summoner Icon in Twisted Treeline? Help Please!

I am really confused on how to get the Summoner Icon that you get when you play Twisted Treeline. I have heard many different ways of getting it and I am confused on how to get it! I have played it 4 times and even Killed the boss of the match! Please Help! Thank you!
