Don't really know how to describe it, so I'll give an example:

"YOU SHALL NOT PASS"

Bot ADC (Emperor's Divide) Mid Mage (Crystallize) Bot Support (Unbreakable) Jungle Fighter/Tank (Pillar of Ice) Top Fighter (Wind Wall)

Basically, list 5 different champions that have an ability or something of the like in common, then list what each of them would do and the name of the ability that puts them in the team.

As another example:

The Undead Redemption NOTE: Give each of these guys a Guardian Angel.
