What is the most embarrassing flash you have ever done?

Here is mine:

Let me paint the picture. I am Wukong top. I am doing pretty well. I think I was 8-3-5 or something close to that. I already had my Black Cleaver, Hydras, and a defensive item. Life was good, except my team initiated a team fight without me and all died. (Where was I? I don't really remember. Probably split pushing.)

So the enemy team is fairly low on life and they decided to Baron. The pit was already warded. It was my time to shine. I was going to wait until Baron was almost dead, flash in, ult, get a penta and the Baron buff. I was going to be the hero. Glory and honor would be mine.

But I botched up the flash, wasted my ult (as depicted in the picture.), and I don't remember if I died or not. I don't think I did. At least my team was nice enough not to rage on me.
