Piano player here, will confirm that it physically feels about the same, however psychologically it's a lot more stressful because in my Piano song if I miss one note I can keep playing, but when I miss that one Zhonya's active at 30 minutes into the game.... I'm still waiting to respawn to keep playing.....

As someone who has experience with the piano; i can confirm this to be 1000000% accurate.
I just use a mouse for slots 5, 6, 7
[zombie-brand-facepalm] Why would you ever have an active on 6 or 7? I have never seen a build with that many active items. You do realize you can swap them over right? And you have enough time as you auto path out of the base to fix it...
Hahahha as someone who struggles heavily with number 6 (and i have to press it twice due to it being RIghteous Glory) i totally feel this.

That's why I rebind my trinket to T and shift over item slots 5-7
And my friends wonder why I hate getting more than 2 activatable items.
