Once I accidentally flashed and decided I wasn't going to let it go to waste and all in'd on the enemy bot lane. Somehow I survived and Jinx got a double.

When you're hands are off and you're one key over so instead of smiting a monster you end up flashing at it.
A couple days ago I was watching EU LCS. I forgot which game it was, but 
flashed while clearing the raptor camp, while the camera was centered on her, while there were no enemies in the area. The audience giggled and the casters tried to avoid noticing it, but everyone saw. It must have been so embarrassing though.
I don't always flash away, but when I do, it's into their jungler.

I do that all the time. When i do something incredibly dumb i just all chat: "No one saw that" or something like that
Once someone said "press D to dance" and I pressed D. I NEED MENTAL HELP!
A friend of mine did this a game or so ago while leashing Red to me... I didn't let her forget it for the entire game.
I always make sure to throw a hook or some kind of CC afterwards to play off like I tried to do something.
"Flash" "Face-palm" Well I'm probably screwed now.
Whenever this happens to me I man up and announce it in all chat.
