And it's even worse for Sion, because the game likes to say "Hey everyone look, Sion just ulted" when you press R.

I had a similar experience.
I was playing 
, and we were doing pretty well, I had my

, along with some other items, and we were doing good.
We were on our way to make a final baron kill to push the last bit into their base and end. We were blue side, and as I was coming up from the bottom I thought it would be a good idea to ward ahead. Well I had 
so it would be simple right? No.
I hit 3 instead of 4, using up my before the fight started.
This earned me a couple of ?'s, but I was more concerned about my own dumb mistake. I felt bad and decided to ward baron like I intended. However my tiltedness caused me to hit R instead. I fired my ult in the completely wrong direction and then decided to take the time to apologize to my team in chat for being dumb all of a sudden, and that I wasn't overconfident or suddenly deciding to throw.
We won the game anyway, as the enemy was at baron and we stole the baron and aced them, but I felt really bad after that game for a while. >.>
I like to ping my ult time to tell my team when it will be up. A lot of times i would miss my alt button and activate my ult... Eventually i disabled hud clicks
