Does it change as they change form? Or does it only apply to their original forms.

This mastery has been tested with accuracy to be safe to say that the mastery does change with their form. Jayce is hard to test this as because he gains bonus defenses in Hammer Stance. Nidalee is better suited to test this fact because only her movement speed gets changed. At level 6 Nid in human form took exactly 80 damage. Upon returning to lane in cougar form the tower did only 78 damage. So the mastery does change with form changing champions.

Everybody be leaving out Kayle in this discussion. Her E changes the mastery as well.

I will say with 95% confidence this changes with their form.

Because of the way Hydra and Moonflair Spellblade works on these champions (tested against bots, lol): these item's actives/ passives only work in the appropriate form.
