can anyone tell me why I didn't get a S+? My best guess is because I used sivir who I don't have mastery 5. Another guess is my farming sucked in the beginning with a total of 170 cs at the end of 27 minutes.

I believe cs is taken into account.

CS and the other huge thing that keeps adc's away from S or better WARDS pinks are prob 90% of the reason an adc doesnt get an S and its not just placing the ward its also did it help in any way

Grades are always relative to other players who play the same champion in the same role. Mastery level is not taken into account.

Take down the turrets, if you chase kills and don't meet the objectives your grade goes down. The system is set up so that you have to help your team win to get a good grade (it is actually a lot easier to get S in support roles) that prevents people from getting a good grade just bc they have a bad lane opponent they can kill over and over and not have to accomplish anything else. (or perish the thought have a friend feed so they can scam a high grade. . . )

S grade is more than just kills. It's kill participation. It's creeps. It's overall damage (a Lux can't just sit and KS every kill and expect S). S grade is there for doing extremely well compared to the global average. It's not just some freebie to be given out.
