Why do I feel that some of my commands are completely ignored??

Sometimes I use my A click and then left click and my character keeps wondering around doing nothing. I feel like after a certain amount of a clicks, there's a bug were a certain one just completely gets disregarded. The lowest my FPS drops is from 120-117 and my MS is always a constant 55.
