I use my pinky on Q, ring on W, middle on E, index on R and I use my thumb on F for flash then swivel it to D whenever it's necessary. I recently found out I'm the only one that does this but is this not more efficient? If not then why the flip not?

I use my index for D/F, It's whatever is comfy for you and give you a personal fastest response time. If you feel like you need to be faster, try different positions, or try re-configuring your keys ingame

I use a Logitech G13 so I remapped things so my hand is comfy, but basically have fingers on QWER and thumb on space (which is my attack move button). My Index moves between 45tydf depending on what is needed, and my thumb sometimes moves when I need to level up an ability or ping something, otherwise it's mostly on my attack move button.
I know some people who have buttons on the side of their mouse, so they rebound control to one of those, or they put their trinket or active items on it as well. So long as it works for you, it's probably fine.
