this bug is probably 5 years old and it still wasnt fixed, do riven mains have to whine like (right through the counter strike ) to get it fixed ? i dont get it i left the game and came back multiple times and it was always still there . i'm 100% sure i dont mess up my clicks or buttons and it happens so often and its so fucking annoying and frustrating to lose lane because of it. It's not a bug. Read the ability.

"Riven will target the unit your cursor is hovering over, or if no targets are present, Riven will simply strike the direction she is currently facing."

You're simply not hovering the enemy, while her actual model is facing a different direction. Since it goes in the direction her model is facing, you'll end up backwards if you don't have your cursor on the target.

Yeah, you can be facing forward and your model will immediately turn the other way and you basically backflip Q. Personally I think they should have just made her Q go in the direction of the cursor. It would make so many things easier. For example 3Qing over walls >_>; Even BoxBox can miss wallhops because of that shit.
