I took 2 mid towers and helped with other towers around the map, and I helped with dragon

That's a low cs? There was one other person in the game with higher. Also, I was constantly getting ganked by Diana and Poppy so there wasn't a lot of time focusing on cs

Kill participation was 54%, but KDR alone is nothing... How many wards did you drop/destroy? How many objectives did you take? (towers/inhibs/JGMonsters) How much damage did you deal/receive/negate? etc.......

I fairly certain this rating system is due to recent performance of that champion accross the server. I think it is similar to the mastery system in World of Tanks. To get "Ace" mastery, you need to earn more experience per battle than 99% of the player base server wide during the past 7 days.

If this is true for League, you may get S or S+ for games with worse performance (less KDA) in other weeks or months on the same champion. I'm not sure what the metrics are, but I would imagine kill participation, ward stats, structure damage, CS, and KDA all play a role in your rating.

Again, just conjecture, but logical imo.

it is based off the performance of other players of that champ.

It shows that Cass is an easy champ to get good stats on.

Your cs killed you, 102 cs at 28 minutes is very low. Not sure what your ward numbers and other stats were either, but I guarantee the cs alone just destroyed your grade.
